Associations between Clinical Characteristics and Plasma BDNF Levels of Panic Disorder

공황장애의 임상적 특성과 Brain-Derived Neurotrophic Factor 농도와의 관계

Abstract

Objective : Brain-derived neurotrophic factor (BDNF) is implicated in the pathophysiology of several neuropsychiatric disorders. However, there are few studies on BDNF of panic disorder. In this study, we investigated plasma BDNF levels in patients with panic disorder, and evaluated whether there are associations between clinical characteristics of panic disorder and plasma BDNF levels.

Methods : We included 110 patients with panic disorder and 110 health controls in the current study. Plasma BDNF levels were measured by the enzyme-linked immunosorbent assay (ELISA). Plasma BDNF level differences were evaluated according to the clinical characteristics, such as duration of illness, recent stressful life event, agoraphobia, and insomnia.

Results : The mean plasma BDNF levels of patients with panic disorder were significantly lower, as compared with those of controls (192.50 pg/mL vs. 693.75 pg/mL, t=8.838, p<0.001). The mean plasma BDNF levels of patients who had recent stressful life events were significantly higher, as compared with those who did not (269.79±358.96 pg/mL vs. 136.94±187.06 pg/mL, t=-2.525, p=0.013).

Conclusion : These results suggested that BDNF plays a potential role in the pathophysiology of panic disorder.

공황장애는 DSM-IV에 의하면 극심한 공포와 자율신경계 기능의 항진 증상이 갑작스럽게 발생하여 수분이내에 최고조에 이르고 그 시간 동안 13가지 생리적, 인지적 증상 중 4가지 이상의 증상이 나타나며, 심각한 예기 불안이 발생하거나 회피 행동과 같은 비적응적 행동을 보이고 사회적 기능의 저하가 야기되는 것을 특징으로 하는 질환이다.1,2 최근 역학조사에 의하면 공황장애는 평생 유병률이 3.5%에 이르며 사회활동이 왕성한 30
~40대에서 호발 한다고 알려져 있다.3,4,5,6 이 질환은 우울증이나 물질의존과 같은 정신질환과 동반하기 쉬우며, 일부 환자에서 심혈관질환과 같은 신체질환의 위험을 증가시킬 수 있다.7 또한, 선택적 세로토닌 재흡수 억제제(specific serotonin reuptake inhibitors : SSRIs) 계열의 약물들이 이 질환의 치료에 매우 효과적임에도 불구하고, 환자들의 60~70%는 치료 반응(treatment response)이 충분하지 않아 만성화의 위험성이 있다.5
하지만 임상현장에서 많은 관심을 요하는 질환임에도 불구하고, 공황장애의 신경생물학적 요인에 대한 이해는 아직 부족한 실정이다. 이전 연구를 살펴보면, 신경해부학적 측면에서 편도(amygdala)를 중심으로 한 공포회로(fear network)의 이상에 대한 가설이 제기되었으며,9,10 norepinephrine과 serotonin을 비롯한 여러 신경전달 물질의 이상,11,12,13 GABA-Benzodiazepine receptor complex binding의 감소,14,15 HPA axis의 변화16 등이 알려져 있다.
최근에는 Brain-derived neurotrophic factor(BDNF)와 공황장애와의 관련성이 관심을 받고 있다. BDNF는 신경세포의 성장, 분화, 시냅스 연결(synaptic connectivity)에 관여하며 신경 세포의 수정 및 생존 등에 관여하는 주요 신경영양인자(neurotrophin) 중 하나로 중추 및 말초 신경계에 매우 중요한 역할을 한다는 주장이 있다.17,18 또한, 조현병, 양극성 정동장애, 주의력결핍 과잉행동장애 등의 다양한 정신질환과 BDNF의 연관성이 보고되었다.19,20,21 BDNF는 대뇌에 많은 양이 분포하고 있으며, 뇌뿐만 아니라 체내의 여러 곳에 분포되어 있다.22,23 동물실험에서 BDNF가 혈액뇌장벽을 통해 혈류에서 뇌로, 뇌에서 혈류로 이동하는 것이 가능함을 설명하였다.24,25 따라서 혈중 BDNF 농도는 많은 부분이 대뇌의 신경세포에서 생성되는 농도에 의해 결정된다고 할 수 있다. 이런 점을 바탕으로 주요우울장애를 포함한 여러 질환을 가진 환자를 대상으로 혈중 BDNF 농도를 측정함으로써 대뇌의 BDNF 활성을 간접적으로 측정하는 임상 연구들이 최근 진행되고 있다.
공황장애의 병태생리와 BDNF의 관련성에 대하여 소수의 연구들만이 진행되었는데, 그 중 Kobayashi 등26은 BDNF 농도와 공황장애의 치료반응의 관련성을 조사하였고, Lam 등27은 유전자 다형성에 관한 연구를 발표하였다. 하지만 대부분의 연구들이 비교적 증상의 정도가 경한 소수의 공황장애 환자들만을 대상으로 혈청에서의 BDNF 농도를 측정한 경우가 대부분이었다.
본 연구는 공황장애 환자를 대상으로 혈장 BDNF 농도를 측정한 후, 공황장애의 임상적 특징과 BDNF 농도와의 관련성을 조사함으로서 공황장애의 병태생리에 대한 기초자료를 얻고자 함을 목적으로 하였다.

대상 및 방법

연구 참여자
대학병원 정신건강의학과에 내원한 환자들 중, Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV-TR)1에 근거한 구조화된 면담(Structured Clinical Interview for DSM-IV Patient Version ; SCID-PV)28을 통해 공황장애로 진단된 110명(남 51명, 여 59)을 대상으로 하였다. 광장공포증의 동반 유무와 상관없이 광장공포증을 동반한 공황장애와 동반하지 않은 공황장애 환자를 모두 참여시켰다. 정신과적 동반 질환으로 조현병, 주요우울장애, 양극성정동장애, 물질 관련 장애 등의 정신질환을 가진 경우는 대상군에서 배제하였다. 또한, 해밀턴 우울증 척도(Hamilton Depression Rating Scale, 이하 HAMD)29에서 15점 이상을 나타낸 환자는 배제하였다. 환자군은 모두 정신과적 약물을 복약하기 전 시점에 연구에 참여하였다.
정상 대조군은 건강진단을 위해 대학병원에 내원한 일반인들 중 구조화된 면담(Structured Clinical Interview for DSM-IV Non-Patient Version ; SCID-NPV)30을 통해 특별한 정신질환이나 신체질환이 없는 대상자를 무선표집하였다(총 110명, 남 66명, 여 44명). 대상자들에게 연구와 관련된 모든 과정들에 대해 상세하고 충분한 설명을 실시한 후에 서면 동의를 받았으며, 본 연구는 전북대학교병원 및 고려대학교병원 임상시험윤리위원회(IRB)의 승인을 받았다.


피험자들에 대한 정신과적 면담 및 임상적 평가는 정신건강의학과 전문의가 시행하였다. 다음과 같은 변인들을 조사하였다 ; 인구학적 변인(연령, 성별, 체중, 신장 등) 임상적 특성(발병연령, 유병기간, 발병 전 6개월 이내 스트레스 유무, 광장공포증, 수면장애 등). 공황장애는 일반적으로 완전 관해(full remission)를 위해 최소한 1년 이상의 약물 치료가 권고되고 있다.31 또한 발병 후 첫 6개월~1년 사이의 임상증상이 회복(recovery)을 예측하는데 가장 중요한 인자로 알려져 있다.32 따라서 본 연구에서 유병기간은 12개월 미만과 12개월 이상으로 구분하였다. 스트레스 유무는 Holmes과 Rahe33이 개발하고 Hong과 Jeong34이 국내표준화한 사회 재적응 평가척도(Social Readjustment Rating Scale, SRRS)를 통해 평가되었다.
혈장 BDNF의 측정을 위해 10 mL lithium heparin 진공 시험관에 혈액을 채취하였다. 정상 대조군의 경우 구조화된 정신과적 면담과 임상적 평가 및 혈액채취가 이루어졌다. 임상적 평가로는 벡 우울 척도(Beck Depression Inventory, BDI)35와 상태불안척도(State-Trait Anxiety Inventory, STAI)36가 사용되었으며 임상적으로 유의미한 우울, 불안을 나타낸 경우 제외하였다. 공황장애 증상의 심각도를 평가하기 위해 한국판 공황장애 심각도 척도(Panic Disorder Severity Scale, PDSS),37 해밀턴 우울척도검사(Hamilton Depression Rating Scale, HAMD),29 해밀턴 불안척도검사(Hamilton Anxiety Rating Scale, HAMA)38 등의 척도를 사용하였다.

효소면역측정법에 의한 BDNF 측정
BDNF의 측정은 영국 R&D사의 DuoSet ELISA Development System(Catalog DY248)를 통해 이루어졌으며, 회사에서 추천하는 buffer, diluents 및 substrate를 사용하여 측정하였다. 측정과정을 요약하면, Capture antibody를 phosphate buffered saline(PBS)에 2 μg/mL의 농도로 희석한 후, 96 well ELISA plate에 100 μg씩 각 well에 넣고 wrap으로 싼 후 실온에 18시간 동안 보관하였다. 각 well에 400 μg의 washing buffer(0.05% Tween-20 in PBS)를 첨가하여 반복 세척(3회)하고, 각 well에 blocking buffer(1% BSA, 5% Sucrose in PBS)를 300 μg씩 첨가한 후 37℃에서 1시간 동안 blocking시켰다. 다시 같은 방법으로 세척한 후 가검물(samples)과 표준시료(standards) 100 μg씩을 각각 첨가한 후 실온에서 2시간 동안 반응시켰다. 동일한 방법으로 세척한 후 2차 항체를 diluent buffer(0.1% BSA, 0.05% Tween-20 in TBS)에 25 ng/mL으로 희석하여 각 well에 100 μg씩 첨가하고 실온에서 2시간 동안 반응시켰다. 다시 위와 같은 방법으로 세척하고, streptavidin conjugated to horseradish-peroxidase를 diluent buffer에 1 : 200으로 희석하고, 각 well에 100 μg씩을 첨가하여 20분간 실온에서 암반응시켰다. 동일한 방법으로 세척한 후, 각 well에 substrate solution을 100 μg씩 넣고 20분간 상온에서 암반응 시키고, 2N H2SO4 50 μL로 발색 반응을 정지시킨 뒤, ELISA reader(Bio-tak insrument)를 사용하여 450 nm의 흡광도에서 반응의 강도를 측정하였다. 각 plate의 BDNF 농도는 각각의 standard curve의 dilution factor를 사용하여 보정하였다. BDNF의 농도는 2회씩 측정한 후 그 평균값을 구하였다.

통계 분석
연령과 체질량지수(Body Mass Index ; BMI) 등의 인구학적 자료와 BDNF 농도와 같은 연속형 변수의 그룹 간 비교를 위해 독립표본 T-검정(independent t-test)을 실시하였으며, 성별과 같은 범주형 변인(categorical variable)의 그룹간 비교를 위해 χ2 검정(chi-square test)을 실시하였다. 공황장애 환자들에서 임상적 특성에 따른 BDNF 농도를 비교하기 위해 독립표본 T-검정(independent t-test)을 실시하였다. 또한, 집단별의 BDNF 농도의 분포를 확인하기 위해 시그마플롯 프로그램을 사용하였다. 본 연구의 통계분석은 윈도우즈용 SPSS (Statistical Package for Social Sciences) 15.0을 통해 이루어졌다.

인구학적 및 임상적 특성
환자군과 정상 대조군 사이의 인구학적 및 임상적 특성은 Table 1에 제시하였다. 환자군의 평균연령은 41.05세였고, 남녀의 비율은 각각 51명(남자, 46.4%), 59명(여자, 53.6%)이었으며, 체질량지수의 평균은 23.02 kg/m2였다. 정상 대조군의 평균연령은 37.9세, 남녀의 비율은 각각 66명(남자, 60%), 44명(여자, 40%)이었으며, 체질량지수의 평균은 22.54 kg/m2이었다. 환자군과 대조군에서 인구학적 특징에 유의미한 차이는 없었다.
환자군의 임상적 특징을 살펴보면, 공황장애의 발병 시 평균 연령은 약 38세였고, 유병기간은 평균 약 35개월이었다. 각 임상척도의 결과를 살펴보면, PDSS는 17.99±3.84, HAMD는 10.43±5.21, HAMA는 20.98±4.88이었다(Table 1).

혈장 BDNF 농도
혈장 BDNF의 평균 농도는 공황장애 환자군(192.50±278.84 pg/mL)에서 정상 대조군(693.75±525.46 pg/mL)에 비해 유의미하게 낮았다(t=8.838, p<0.001)(Table 2).

공황장애의 임상적 특성에 따른 혈장 BDNF 비교
임상적 특성에 따라서 분류하여 평가한 바로는, Table 3에 제시된 바와 같이, 공황장애 발생 6개월 이내에 주요 생활사건이 있었던 환자군과 주요 생활사건이 없었던 군의 사이에는 혈장 BDNF 농도에서 유의미한 차이를 나타내었다(t=-2.525, p=0.013). 두 군의 혈장 BDNF는 주요 생활사건을 발병 직전에 경험한 경우는 269.79±358.96 pg/mL, 그렇지 않은 군은 136.94±187.06 pg/mL이었다.
이와 더불어 유의미한 값은 아니지만, 유병기간이 긴 환자군에서 유병기간이 짧은 환자군에 비하여 BDNF 농도가 감소하였고 광장공포증이 있는 환자군 및 수면장애가 동반된 환자군에서 그렇지 않은 환자군보다 BDNF 농도가 감소하였음을 발견하였다(Table 3). 또한 BDNF 농도와 각 임상척도(PDSS, HAM-A, HAM-D)간의 상관분석 결과 유의미한 연관성을 나타내지 않았다(표로 제시하지 않음).

본 연구는 공황장애 환자군에서 정상 대조군보다 BDNF 농도가 유의미하게 감소되어 있음을 발견하였다. 또한 공황장애 환자군의 임상적인 특성과 관련하여 연구를 진행하였는데, 최근 주요 생활사건이 있었던 환자군이 그렇지 않은 환자군에 비하여 BDNF 농도가 유의미하게 증가되어 있음을 발견하였다.
본 연구에서 나타난 공항장애 환자군의 혈장 BDNF 농도의 감소는 말초 혈소판에서의 분비저하를 반영하는 것일 수 있다. 최근 정신과적 질환을 가진 환자에서 혈장 BDNF를 측정한 많은 연구들에서 인간 및 다른 포유동물의 혈청에서도 BDNF가 발견되고 있으며, BDNF는 본질적으로 혈소판에 저장된 상태에서 활성화 또는 응고화 과정을 거친 후 혈장으로 분비된다는 주장이 지배적이다.39,40,41,42 더욱이 Larghissi-Thode 등43과 Musselman 등44은 우울증 환자들에서 혈소판 활동의 이상이 발생한다고 보고한 바 있다.
다른 의미로 중추신경계의 BDNF 수치를 말초기관인 혈장 BDNF를 통해 유추해 볼 수 있음을 시사하고 있다. 대뇌는 BDNF가 주로 생성되는 곳인데 주요우울증 환자의 경우 대뇌에서 BDNF의 생성과 활성이 감소하고, 핼액뇌장벽을 통한 말초혈액으로의 이동이 감소하게 된다. 주요우울장애 환자를 대상으로 한 연구에서 혈소판에 저장된 BDNF 농도가 정상군에 비해 감소되어 있었으나,45 말초혈액에서 BDNF mRNA는 정상군과 주요우울증 환자 사이의 유의한 차이를 보이지 않았다.46 이러한 맥락으로, 공황장애 환자에서 혈장 BDNF 농도의 저하는 말초혈액에서 BDNF 생성이나 분비의 저하보다는 주로 대뇌 BDNF 생성과 작용 저하의 결과로 추정해 볼 수 있다.
다른 가능성은 세로토닌의 이상으로 인해 BDNF가 감소되었을 수 있다는 점이다. 세로토닌 수용체의 작용은 cyclic Adenosine Mono-Phosphate(cAMP), phosphatidylinositol, 칼슘 등과 같은 신호 전달체계와 관련되어 있으며, BDNF 유전자 발현에 있어 Cyclic AMP response element binding protein(CREB)이 중요한 역할을 하는 것으로 알려져 있다. CREB은 protein kinase C(PKC)와 Ca2+/calmodulin dependent protein kinase의 작용에 의해서도 활성화되는 것으로 알려져 있으며,47,48 항우울제를 투여하는 경우 NMDA 수용체 자극을 통해 Ca2+-dependent kinase를 조절할 수 있다고 알려져 있다.49 이외에도 공황장애를 포함한 다양한 불안장애 환자들에서 세로토닌과 그 대사물인 5-hydroxyindole acetate의 감소가 보고되고 있다.50 선택적 세로토닌 재흡수 차단제는 대부분의 불안장애에서 주 치료제로써 사용되고 있으며, 그 치료효과와 기전도 밝혀진바 있다.51,52,53 따라서 위의 결과들을 종합해볼 때, 공황장애 환자에서 세로토닌 시스템의 이상으로 인해 BDNF의 농도 감소를 나타냈을 가능성이 있다.
본 연구의 결과에서 공황장애 환자의 BDNF 농도와 임상적 특성 사이의 관계를 살펴보면, 공황장애 발병 직전에 스트레스가 될 만한 생활 사건이 있는 환자들에서 그렇지 않은 군에 비해 유의미하게 높은 혈장 BDNF 농도를 나타내었다. 우선, 질병의 만성화 여부가 BDNF 농도와 관련이 있을 것으로 생각할 수 있다. 발병에 앞서 최근 스트레스가 될 만한 생활 사건이 있었다는 것은 현재 나타나는 공황장애 증상이 만성화로 인한 영향보다는 스트레스 사건에 대한 반응성임을 시사하는 소견이라고 할 수 있다. 이에 반해, 비록 유의한 값은 아니었지만, 유병기간이 긴 환자군, 광장공포증을 동반한 환자군, 수면장애를 동반한 환자군에서 그렇지 않은 군에 비해 BDNF 농도가 더 낮은 경향을 나타내었다. 유병기간이 길어질수록, 동반증상이 많을수록 환자는 더욱 심한 증상을 경험하게 되며 질병의 만성화가 진행될 수 있다. 이러한 점이 반영되어 유병기간이 더 긴 환자군, 광장공포증 동반 환자군, 수면장애 동반 환자군에서는 상대적으로 더 낮은 BDNF 농도를 나타내었으며, 최근 생활 사건이 있는 환자군은 그렇지 않은 군에 비해 상대적으로 높은 BDNF 농도를 나타내었을 것이라 추론해 볼 수 있다. 또한, 기존의 동물연구에서 Spina 등54과 Smith 등55은 BDNF의 분비가 스트레스에 의해 조절되고 BDNF 자체가 급성 스트레스에 대한 방어기능을 가질 수 있음을 보고하였으며, 급성 스트레스가 prefrontal cortex와 cingulate cortex에서 유의미한 일시적인 BDNF의 mRNA level 증가를 나타내었다는 연구도 있었다.56,57 이를 통해, 급성 스트레스에 대한 단기 방어기제로서 혈장 BDNF 농도의 상대적 상승이 있었을 것을 추론해 볼 수 있을 것이다. 하지만, BDNF 농도와 관련된 기존의 연구들은 대부분 주요우울장애 환자와 스트레스간의 연관성에 대해 이루어져 왔으며, 이 경우 최근 스트레스와 연관되어 BDNF의 농도는 감소한 결과를 나타내고 있으며,58,59 BDNF 농도와 공황장애의 연관성에 대한 연구는 매우 부족한 실정이다. 따라서, 공황장애 환자에서 스트레스와 BDNF 농도 사이의 연관성에 대해서는 추가적인 많은 연구가 필요할 것이다.
본 연구에는 다음과 같은 몇 가지 제한점이 있다. 첫째, 임상적 특성의 지표로서 최근 생활사건, 유병기간, 광장공포증 및 수면장애의 동반 유무 등 비교적 소수의 지표만을 사용하였다. 추후 공황장애의 더 많은 임상적 특성을 변수로 하여 BDNF 농도를 조사할 필요가 있다. 둘째, BDNF 농도의 측정이 2차례 이루어졌으나 두 측정 사이의 시간이 짧아 시간 경과에 따른 추이를 살펴보지 못했다. 따라서 향후 연구에서는 종단적인 연구 수행을 통해 BDNF 농도의 시간적 변화를 살펴볼 필요가 있다. 특히 최근의 스트레스가 될 만한 생활 사건이 있었을 경우 시간의 경과에 따른 결과를 측정하는 것이 스트레스 요인과 관련된 공황장애의 병태생리를 이해하는데 도움이 될 수 있을 것이다. 셋째, 말초혈액의 BDNF 농도를 측정하는데 있어서 어떤 종류의 검체를 사용할 것인지에 대한 일괄된 결론은 없다. 본 연구에서는 혈장에서 BDNF의 농도 측정을 실시하졌지만 혈청, 혈장, 전혈 등 다양한 검체에서의 BDNF의 농도를 확인할 필요가 있을 것으로 보인다. 마지막으로, 본 연구 결과에서 환자군과 대조군 사이에 연령, 성별에 따른 차이를 나타내지는 않았지만 환자군의 평균 연령이 다소 높고 여성의 비율이 높은 경향을 보였다. BDNF농도와 연령과의 연관성에 대한 기존의 논문들에서는 연령에 따른 감소를 보고한 연구들이 주로 있으나,60 연관성이 없음을 보고한 연구들도 일부 발표되었으며,61 BDNF 농도와 성별과의 연관성에 대해서는 주로 연관성이 없음을 보고하고 있다.62 이처럼 BDNF 농도와 연령, 성별에 따른 연관성에 대해서는 명확한 결론을 제시하지 못하고 있다. 추후 연구에서 BDNF 농도와 연령, BDNF와 성별 간의 연관성에 대해 확인할 필요가 있을 것으로 보인다.

본 연구에서 공황장애 환자의 혈장 BDNF의 농도는 정상대조군에 비해서 유의미하게 낮은 수치를 나타내었다. 또한 공황장애 발생 6개월 이내에 주요 생활사건이 있었던 환자군과 주요 생활사건이 없었던 군의 사이에는 혈장 BDNF의 농도에 유의한 차이를 나타내었다. 이는 BDNF의 농도가 공황장애의 병태생리에 관련이 있고, 이와 더불어 스트레스 및 질병의 만성화와 관련이 있으며, 더 나아가 공황장애의 생물학적 지표로 고려될 수 있을 것으로 생각된다.
