Molecular Genetics of Anxiety Disorder

불안장애의 분자유전학적 이해

Abstract

Anxiety disorder is likely caused by an interaction of multiple loci in brain, rather than a single locus. Hyperactive neurotransmitter circuits between the cortex, thalamus, amygdala, and hypothalamus are responsible for production of anxiety symptoms. Familial studies performed on anxiety disorder suggested that anxiety disorder should be caused by genetic etiology. Numerous linkage and association studies showed different genetic loci of anxiety disorder. Candidate genes have been focused on important neurotransmitters, neuropeptide, or genes affecting neuronal growth, development, protection or apoptosis. Anxiety disorder has various symptoms and comorbid diseases in family or proband. Therefore, further studies focused on symptomatic dimension of anxiety disorder or responses to drugs are required.

서     론


  
불안장애는 뇌의 단일부위보다는 여러 부위가 서로 상호작용하여 발생한다. 대뇌 전두엽(prefrontal cortex), 시상(thalamus) 및 시상하부(hypothalamus), 해마(hippocampus), 편도(amygdale), 청색반(locus coeruleus) 등이 불안장애와 관련되어 있다고 보여지며, 이 부위들에서 생성되는 세로토닌(serotonin), 노르아드레날린(noradrenaline), 도파민(dopamine), gamma-aminobutyric acid (GABA), 벤조디아제핀(benzodiazepine) 수용체, 부신피질 호르몬 등 여러 가지 신경전달물질들의 과잉 또는 결핍이 불안장애의 증상을 유발한다.
   불안장애의 원인으로서 심리적 원인, 환경적 원인 뿐만 아니라 유전적 원인을 포함한 생물학적 원인 등이 고려되어 왔다.
   그 동안 불안장애의 분자유전학적 연구가 다양한 방법으로 진행되어 왔다. 이를 크게 나누어 보면, 가계 내에서의 불안장애 및 불안장애 증상과 유전자와의 관련성을 보는 연관 연구(linkage study)와 불안 장애 환자와 정상인에서 유전자의 차이점을 알아 보는 관련 연구(association study)가 있다. 관련 연구는 주로 불안장애와 관련이 있다고 보여지는 신경전달물질 및 호르몬 생성 및 대사에 관여하는 유전자를 중심으로 이루어졌다. 본 논문에서는 불안장애와 관련된 현재까지의 유전학적 연구를 각 질환 별로 고찰해 보고자 한다.

본     론

   정신질환의 분자유전 연구 방법 중 연관연구는 여러 명의 환자가 있는 가계를 대상으로 유전자와 질환간의 연관 정도를 분석하는 것으로 가족 내에서 유전자 표지자(genetic marker)와 질병의 표현형간의 co-segregation을 찾는 것이다. 이와는 대조적으로 관련연구는 환자군과 정상대조군에서 특정 대립유전자의 발현빈도 차이를 분석하는 것으로 후보유전자(candidate gene)를 중심으로 이루어진다. 불안장애의 후보유전자로는 불안장애와 관련이 있다고 보여지는 신경전달물질(dopamine, serotonin, GABA, catecholamine)의 수용체(receptor)와 수송체(transporter) 유전자 및 그 대사관여물질(monoamine oxidase (MAO), catechol-O-methyltransferase(COMT)) 유전자, 신경단백질(neuropeptide;corticotropin releasing hormone(CRH), vasopressin, oxytocin, somatostatin, neuropeptide Y, Cell surface proteins that bind cholecystokinin(CCK)) 유전자, 신경 성장, 발달 및 괴사(apoptosis)에 관여하는 유전자 등 다양한 유전자들이 있다.
   그러나, 불안장애는 가계 및 환자에서 다른 정신질환과 공존하는 경우가 많고, 불안장애의 종류에 따라 증상이 다양하여 연관 연구에서 유전자 표지자 탐색 및 관련 연구에서의 후보 유전자 선택에 어려움이 있다. 따라서 불안장애를 하나의 진단으로 분류한 유전적 접근은 불가능하며, 각 질환별로 유전 연구가 진행되어 왔다. 그러므로, 아래와 같이 대표적인 불안장애들에 대한 유전적 연구들을 조사해 보고자 한다. 연구 결과는 연구자에 따라 다양하게 보고되었으나, 본 논문에서는 의미 있는 결과를 도출한 연구들을 중심으로 고찰하고자 한다.

강박장애(Obsessive-Compulsive Disorder;OCD)의 유전학적 연구
   OCD가 유전적 소인이 있다는 근거는 이미 1929년부터 쌍생아 연구(twin study)를 통해 이루어져 왔으며, 소아 OCD는 45
~65%, 성인 OCD는 27~47%에서 유전적 소인이 있는 것으로 추측된다.1
   이 외에 가계 연구를 통한 segregation analysis에서도 OCD는 상염색체 우성(autosomal dominant) 양상을 갖는 유전적 소인이 있는 것으로 추측되었다.2 그러나, OCD는 다양한 증상과 동반질환을 가진 증후군(syndrome)으로 보는 견해가 많으며,3,4 이로 인해 발병 연령 등 OCD의 임상 양상을 중심으로 이루어진 유전 연구들도 있었다.5
   Hanna 등6은 OCD 환자의 가계 조사를 통해 OCD 환자의 일차 친척에서 OCD의 발병율이 높다고 보고하였다. 가계 내 genome scan을 통해 OCD와 9p24가 관련되어 있다는 보고가 있었고, 가계 내 유전적 소인이 있거나 발병 연령이 이른 OCD 환자는 그렇지 않은 환자와는 다른 임상적인 특징을 가지고 있다고 하였다.7,8
   가족 내 haplotype relative risk(HRR)와 transmission disequilibrium test(TDT)를 통해 OCD가 저활성의 COMT 대립유전자와 관련이 있다는 보고들이 있었고,9,10 MAO-A와 관련된다는 보고10,11도 있었다. Mossner 등12은 TDT를 통해 tryptophan hydroxylase(TPH) 유전자가 조기 발병 OCD와 관련이 있다고 보고하였다.
   대조군 연구에서는 주로 세로토닌 관련 유전자들에 대해 이루어졌다. 5-HT2A 수용체 유전자와 OCD가 관련이 있다는 보고가 있었으며,13,14 5-HT1Dβ와는 OCD의 증상과 관련된다는 보고들15,16이 있었다. 세로토닌 수송체 유전자에 대해서는 variable number of tandem repeats (VNTR) 부위와의 관련,17 uncommon coding 부위와의 관련,18 promoter 부위와의 관련19,20 등이 보고되었는데, 특히 Kim 등20은 promoter 부위가 OCD의 증상과 관련된다고 하였다. Denys 등21은 세로토닌 관련 유전자들과 OCD의 임상양상과의 관련성을 보고하였다.
   Hall 등22은 brain-derived neurotrophic factor(BDNF) 유전자와의 관련을 보고하였는데, met66 대립유전자가 OCD에 대해 방어효과가 있다고 하였다. OCD가 연쇄상 구균 감염(streptococcal infection)에 따른 면역질환이라는 가설에 따라 Zai 등23은 myelin oligodendrocyte glycoprotein-4(MOG-4)와 OCD의 연관을 TDT를 통해 분석하여 MOG-4 유전자가 OCD 및 OCD 증상의 심각도와 관련된다고 보고하였다.
   OCD의 임상양상과 관련 유전자에 대한 연구 중 성별에 따른 차이도 보고되었는데, Lochner 등24은 남자 OCD 환자에서는 MAO-A의 EcoRV 유전자 중 높은 활성을 보이는 T 대립유전자와 관련되는 반면 여자에서는 낮은 활성을 보이는 C 대립유전자와 관련된다고 하였다.
   이 외에 gamma-aminobutyric acid type B receptor 1(GABBR1)의 관련성,25 조기발병 OCD와 dopamine D4 수용체(DRD4) 유전자와의 관련성26이 보고되기도 하였다.

공황장애(Panic disorder)의 유전학적 연구
  
가계 연구를 통해 공황장애는 유전적 성향을 갖는다고 알려져 왔다. 연구마다 다르기는 하지만, 대개 30
~40%의 공황장애 환자들이 가계 내 유전적 요인을 갖고 있다고 조사되었다.27 공황장애의 유전학적 연구는 비교적 오래 전부터 이루어져 왔다. Cohen 등28은 1951년에 현재의 공황장애 개념과 유사한 증상을 보이는 증상군을 neurocirculatory asthenia(NCA;anxiety neurosis, effort syndrome)이라고 하였고, 가족력이 있으며, NCA 환자의 아버지에서 높은 알코올 의존 유병률을 보인다고 하였다. 또한, Noyes 등29은 불안 신경증(anxiety neurosis)을 갖는 환자의 일차 친척에서 같은 질환의 유병률이 정상 대조군보다 높다고 보고하였다. 한편, Hopper 등30은 유전자에 의한 영향 외에도 공통적인 가족 내 환경 요소의 중요성을 강조하였다. 쌍생아 연구에서는 이란성 쌍생아보다 일란성 쌍생아에서 공황장애의 높은 발병 일치도를 보였다.31 공황장애의 임상아형을 이용한 가계 내 유전적 성향 연구도 활발히 진행되었는데, 특히 Goldstein 등32은 20세 이전에 조기 발병하는 공황장애 환자에서 가족 내 발병률이 현저히 높다고 보고하였다.
   공황장애에 대해서는 그 동안 많은 genome-wide scan을 통한 연관 연구(linkage analyses)가 있었다. 공황장애의 가계 내 유전자 지도(genetic mapping)에서는 다양한 염색체 부위가 공황장애와 연관되어 있다고 보고되었다. 염색체 7p(D7S284633, D7S52134), 13q(D13S77 935, D13S79336), 16p(D16S74934), COMT 유전자가 위치하는 22q(D22S94437) 등이 유의한 관련이 있다고 보고되었다. Logue 등34은 Crowe 등33이 연구한 자료를 재분석하여 7p 외에 16p가 관련되어 있다고 보고하였다. Weissman 등35은 1개 이상의 증후군적 증상을 갖는 환자를 따로 분류하여 분석하였을 때 더욱 높은 연관성을 보인다고 하였다.
   공황장애의 관련 연구(association study)에서는 세로토닌 관련 유전자38,39,40,41 및 COMT 유전자42,43,44에 대한 연구들이 많았다. Woo 등42은 COMT의 met/met 유전자형을 갖는 군이 불안 증상이 심하고 paroxetine에 대한 약물 반응이 나쁘다고 하였고, Perna 등45은 5-HTTLPR 유전자형과 paroxetine에 대한 약물 반응이 관련된다고 하였다. Freitag 등46은 공황장애에서 세로토닌과 노르에피네프린 관련 유전자들의 상호작용을 연구하여 5-HT1A와 COMT 유전자의 상호작용이 공황장애와 관련되어 있을 가능성이 있음을 시사하였다. Domshke 등40은 5-HT TLPR 유전자와 공황장애 환자에서 감정적 자극에 대한 뇌활성과 5-HTTLPR 유전자가 관련되어 있다고 보고하였다.

사회공포증(Social phobia)의 분자유전학적 연구
  
사회공포증의 유전학적 연구는 아직까지 많이 진행되지 못 하였다. Gratacos 등47은 염색체 15q24-26의 interstitial duplication(DUP25)이 공황장애 뿐 아니라 공포증과 관련이 있다고 하였으나 이후의 연구에서는 관련이 없다는 보고를 하였다.48 신경전달물질과의 관련 여부에 대한 연구에서는 COMT와 관련된다는 보고가 있었고,49 Kennedy 등50은 사회공포증 환자의 가계에서 연관연구를 한 결과 도파민 관련 유전자와는 연관이 되지 않는다고 보고하였다. Furmark 등51은 5-HTTLPR의 짧은 대립유전자를 가진 환자가 증상이 더 심하고 불안자극에 대한 편도의 흥분이 더 크다고 하였다.

외상 후 스트레스장애(Post-traumatic Stress Disorder;PTSD)의 분자유전학적 연구
   Freeman 등52은 APOE 유전자가 PTSD의 증상 및 기억 장애와 관련이 있다고 하였다. Feusner 등53은 PTSD와 GABA-A 수용체 beta 3 subunit 유전자와의 관련 연구에서 신체증상, 불안 및 불면, 우울, 사회적 기능 상실 등의 증상과 유전자형이 관련된다고 보고하였다. 도파민 관련 유전자 연구에서는 도파민 수송체 유전자(DAT)와 관련된다는 보고54와 도파민 D2 수용체 유전자(DRD2)와 관련된다는 보고55가 있었다. Lawford 등56은 PTSD 환자에서 paroxetine 투여에 대한 반응과 DRD2 유전자 다형성이 관련된다고 보고하였다. Lee 등57은 최근에 세로토닌 수송체 유전자가 PTSD와 관련된다고 보고하였다. 

범불안장애(Generalized anxiety disorder)의 분자유전학적 연구
  
범불안장애의 분자유전학적 연구는 아직까지 많이 진행되지 못한 상태이다. You 등58은 범불안장애 환자의 세로토닌 관련 유전자 연구에서 일반 대조군보다 5-HTTLPR의 ss 유전자형이 일반 대조군보다 높다고 보고하였다. 그러나, 5-HTTLPR의 VNTR 다형성과 TPH 유전자는 관련이 없다고 하였다. Stein 등59은 5-HTTLPR 유전자형이 범불안장애 환자에서 selective serotonin reuptake inhibitor(SSRI) 계통의 약물에 대한 반응과 관련된다고 보고하였다.
   MAO-A의 단일염기 다형성60 및 VNTR 다형성61이 범불안장애와 관련된다는 보고도 있었다.

결     론

   불안장애는 뇌신경계의 여러 부위가 서로 상호작용하여 발생하며, 세로토닌, 노르에피네프린, 도파민, GABA 등 다양한 신경전달물질들과 관련이 있다. 그 동안 불안장애에 속하는 강박장애, 공황장애, 사회 공포증, 범불안장애, 외상후 스트레스 장애 등에 대해 가계 내에서의 연관 연구 및 신경전달물질들에 대한 관련 연구가 진행되어 왔으나, 아직 뚜렷한 결과를 얻지 못하고 있다. 불안장애는 다른 질환과 동반되거나 증상의 구별이 어려운 경우가 많다. 향후 불안장애 증상에 대한 세분화되고 유전적인 접근이 필요할 것으로 생각되며, 항불안제 반응과 유전자와의 관련성을 알아 보는 것도 간접적으로 도움이 될 것이다.
